The application of Cambodian agarwood in traditional Chinese medicine therapy: from traditional formula testing to modern pharmacological research

Khmer agarwood belongs to the category of "agarwood" in traditional Chinese medicine. It has a warm nature, a pungent and bitter taste, and can regulate the spleen, stomach, and kidney meridians. It has the effects of promoting qi circulation, relieving pain, warming the middle, stopping nausea, and absorbing qi to relieve asthma. Modern pharmacological research has confirmed that its components such as agarwood snail alcohol and chromogen ketone have anti-inflammatory, analgesic, neuroprotective, and other effects. This article analyzes the technical value of Cambodian agarwood in traditional Chinese medicine therapy from three aspects: traditional application, modern pharmacology, and clinical research.

1、 Traditional Applications: Classic Formula Testing and Processing Techniques

Khmer agarwood has long been recorded in traditional Chinese medical texts. The Compendium of Materia Medica states that it is the "first fragrance", while the Compendium of Materia Medica records that it "treats upper heat and lower cold, as well as shortness of breath and shortness of breath". Traditional applications focus on three major areas:

Spleen and stomach regulation

For stomach cold vomiting, bloating and abdominal pain, a combination of cloves and cinnamon is used to make "Chenxiang Warm Stomach Pills", 3 grams each time, taken with warm water, for 3 days as one course of treatment. In traditional Cambodian medicine, agarwood powder is mixed with honey and applied externally to the navel to treat pediatric colic, with an effective rate of 85%.

respiratory system

For asthma and chronic cough, the "Chenxiang Zhike Powder" is made by combining almonds and perilla leaves. It is brewed with boiling water as a tea replacement drink twice a day for 7 consecutive days to relieve symptoms. Vietnamese folk use agarwood tobacco (a mixture of agarwood powder and tobacco) for inhalation, which stimulates the respiratory mucosa through fragrance and assists in the treatment of chronic bronchitis.

Calm the nerves and aid sleep

To address insomnia and anxiety, agarwood tablets are boiled together with lavender and acacia flowers to create a "calming sachet" placed next to the pillow. The agarwood snail alcohol in its volatile oil can regulate gamma aminobutyric acid (GABA) receptors, reducing sleep time by 30%.

2、 Modern Pharmacological Research: Component Analysis and Mechanism of Action

Modern research has revealed the pharmacological effects of Cambodian agarwood by isolating its active ingredients

anti-inflammatory effect

Agarwood snail alcohol can inhibit the NF - κ B signaling pathway and reduce the release of inflammatory factors such as TNF - α and IL-6. Animal experiments have shown that the inhibition rate of mouse ear swelling model is 60%, which is comparable to dexamethasone.

Analgesic effect

Chromogenic ketones inhibit pain signal transmission by blocking calcium ion channels. In clinical trials, the analgesic effect of Khmer agarwood extract on postoperative pain (reducing NRS score by 4 points) was superior to ibuprofen (reducing score by 3 points).

Neuroprotective effect

β - agarwood furan can promote the secretion of nerve growth factor (NGF) and protect brain neurons. Experiments on Alzheimer's disease model mice showed that after 28 days of continuous administration, spatial memory ability improved by 40%.

antitumor activity

2- (2-phenylethyl) ketone in agarwood can induce apoptosis in liver cancer cells, and its mechanism is related to upregulation of Bax protein and downregulation of Bcl-2 protein expression. In vitro experiments showed that the inhibition rate on HepG2 cells reached 50%.

3、 Clinical Research: From Animal Experiments to Human Applications

In recent years, clinical research on Cambodian agarwood has gradually deepened:

Functional dyspepsia

A randomized controlled trial (RCT) included 120 patients, with the treatment group taking orally agarwood extract capsules (0.5 grams three times a day) and the control group taking orally domperidone. After 4 weeks, the symptom improvement rate of the treatment group (85%) was significantly higher than that of the control group (60%), and there were no side effects such as diarrhea and dizziness.

Chronic obstructive pulmonary disease (COPD)

Sixty patients were randomly divided into two groups. The treatment group received routine treatment plus agarwood nebulization inhalation (twice a day, each containing 0.1 ml of agarwood volatile oil), while the control group received only routine treatment. After 8 weeks, the FEV1 (forced expiratory volume in the first second) of the treatment group increased by 15%, and the quality of life score (SGRQ) decreased by 20 points, with better results than the control group.

insomnia

An open label test included 40 insomniacs who wore sachets containing Khmer agarwood essential oil before going to bed for 4 consecutive weeks. The results showed that the patient's bedtime was shortened from 45 minutes to 25 minutes, and the total sleep time was extended from 5.2 hours to 6.8 hours, without drug dependence.

4、 Technological Challenges and Future Directions

Despite its clear pharmacological effects, the clinical application of Cambodian agarwood still faces challenges:

Quality control: There are significant differences in the composition of agarwood from different origins and aroma forming methods, and a unified quality standard needs to be established (such as agarwood snail alcohol content ≥ 5%).

Formulation innovation: Traditional decoctions and pills have poor taste, and new formulations such as sublingual tablets and transdermal patches need to be developed to improve patient compliance.

In the future, technology will develop towards "precision medicine" and "internationalization":

Individualized medication: Predicting patients' metabolic capacity for agarwood components through genetic testing, adjusting dosage and treatment course.

International registration: Conduct clinical trials that comply with FDA and EMA standards to promote Khmer agarwood as an internationally recognized natural medicine.
